Relevant athletic and school-sponsored activity forms can be found below:

• Travel Release Form (to be downloaded, filled out and returned to the sponsor BEFORE the student leaves for an event.  Alternatively you could print the form and scan it or take a picture, then email or send via ParentSquare the signed form to the sponsor).
• Use of School Facilities Agreement Form (to be downloaded, filled out and returned to Mr. Zach Nesbitt - emailing a scan of the completed form is acceptable).
